After lip augmentation, swelling is almost always expected. Especially in the first 24 to 72 hours, the lips may appear uneven, firmer, or significantly fuller than planned. Small hematomas, pressure sensitivity, and a feeling of tightness are also typical. This is usually not a sign of a poor result, but a completely normal biological tissue reaction. The fact that the lips initially feel somewhat harder or small irregularities are palpable can also be temporarily normal. The material needs time to distribute in the tissue. Many judge the result too early and get unnecessarily alarmed. A realistic assessment is usually only possible after a few days, sometimes only after about two weeks.
However, it’s also important to know when abnormalities need to be taken seriously. Less common are temporary nodules, asymmetries, herpes reactivations, or an irregular feel. Rare but crucial are the mentioned vascular complications. That’s why lip treatment should never be viewed as a pure lifestyle or cosmetic service.
If old material is already present in the lips, this should be openly discussed during the consultation. Not every lip can simply be treated “on top.” Sometimes it’s more sensible to first dissolve existing, unclean fillers with the enzyme Hylase and rebuild the area. This is especially true for filler migration, when the material has shifted above the lip contour and an unnatural bulge (“duck lips”) develops.

How long does hyaluronic acid last in the lips?
Duration is individual. As a rough guideline, about 6 to 12 months applies, sometimes slightly shorter, sometimes longer. Lips are very mobile and extremely well perfused, which is why hyaluronic acid is often broken down faster there due to mechanical stress than in more static facial regions. Those with a fast metabolism, lots of facial expression, or who are very athletic may notice shorter duration.
The initial condition is equally important. With very thin lips, even small volume loss is noticed earlier, while well-defined lips can appear “fresh” longer. Product choice also plays a role. A very soft preparation often looks particularly natural but may break down faster than a somewhat more structured, higher cross-linked product. From a medical perspective, it’s not sensible to re-inject too early and too frequently just because the lips are no longer maximally swollen after a few days. Swelling is not the actual result.

Can lip augmentation be reversed?
Yes, unlike permanent materials, hyaluronic acid can be completely dissolved with the enzyme Hylase (hyaluronidase). This makes sense when the result looks unnatural, the filler has migrated into the skin surface, asymmetries disturb the overall appearance, or rare medical indications exist. However, it’s important: Hylase is not a cosmetic eraser for casual use.
Dissolving requires exact medical dosing and experience, as the enzyme can temporarily also affect the body’s own hyaluronic acid. In our practices, we therefore always carefully weigh whether complete dissolution is necessary or whether a targeted, fine correction is sufficient.
Does lip augmentation hurt?
